Effective January 1, 2004, Minnesota Life Insurance Company will replace Metropolitan Life as the insurer for the following UM benefit programs:
Group Term Life Insurance Program, Supplemental, Life Insurance Program, Optional Term Insurance, Group Universal Life Insurance and Dependent Life Insurance Program.
In addition, Minnesota Life will also replace Mutual of Omaha Insurance Company as the insurer of the UM Accidental Death and Dismemberment Insurance Program.

The basis coverage provisions of the Group Term Life, Optional Term Life, Dependent Life and Accidental Death and Dismemberment programs will not change. Premiums for the Group Term Life and Optional Term Life options will decrease. These decreases should be reflected in December 2003 payroll deductions. The amount of the decrease will vary by each individual and is dependent on the plan in which enrolled as well as coverage levels.
December's Optional Term Life program premiums for employees who have elected coverage based on a multiple of salary, will also reflect changes in coverage level that have resulted from preceding salary changes.
Effective January 1, 2004, the Group Universal Life Insurance option will no longer be available to new employees and enrollees. Employees who are currently enrolled in this option will be allowed to continue their coverage under this program through Minnesota Life. However, UM payroll deduction will no longer be available. Minnesota Life will send a special notice to employees enrolled in the Group Universal Life Insurance option. This notice will provide complete information on how accumulated assets and coverage will be transferred.
